The Aftercare program provides support to residents who have recently graduated from The Bridge and their families for 12 months upon the resident's discharge. It is our goal to ensure the permanency and stability of each youth in their home community. Forty-eight percent of youth at The Bridge discharge directly home to family. Aftercare program staff maintain consistent contact with familes and make regular site visits to ensure a the family is connected to the community resources they need to thrive. |
